Free Vaccines for Life Program

One-Time Administrative Fee: $120.00

To be eligible for the Free Vaccines for Life program, you as a pet parent will need to satisfy the following criteria:

1. Pay a one-time administrative fee of $120.00 per pet (nonrefundable-nontransferable).
2. Be current on the following wellness criteria for each individual pet (every year).

• • Comprehensive head to tail Physical Examination as indicated for the age and species/breed.
  • Microchip if not already applied
  • Prophylactic screening Blood work. (Wellness screening)
  • Infectious Diseases Screening Test (as applicable for species and breed)
  • Heartworm and or Fecal Parasite Test (As applicable for species and breed)
  • Be Current on Preventatives for External and Internal parasites included Fleas, Ticks and Heartworms. (As applicable for the species and breed)

Our intention with this plan is to educate and make aware all pet owners that routine health is just much more than only vaccines, making sure your pet is covered and protected as well as to monitor all body function indicators to make sure your pet does not have any pre-existing or ongoing conditions that can be detected, monitored and or treated early on its course.

• As part of the FVFL plan, our practice will provide for all pets enrolled in the program, all the core vaccines for free. If on any specific year all the required criteria are not met, the patient will be dropped form the program automatically. You can always re-enroll any pet.
• All tests, Vaccines and Preventives must physically be bought at our facility to comply with the agreement, this will allow us to compensate for the free services given to your pet while in the program.
• At the annual wellness exam, the veterinarian will discuss the vaccines appropriate for the age and lifestyle of that pet and recommend a vaccination program. The client has the right to refuse vaccines or request additional vaccines that are not listed on this form. Any vaccine not listed or explicitly excluded on this form are not included in the Free Vaccines for Life Program and will carry an additional charge.
• All exam fees, laboratory fees, medications, preventives, and any other charges are mandatory to stay in the program as they are extremely important for the long-term health of your pet.
• Client must pay for the doctor’s examination fee to receive the free vaccines.

Vaccines that are included in this form include:

• Dogs – DA2PP, Rabies, Bordetella. (Influenza Vaccine or Leptospira Vaccines are not part of the FVFL plan)
• Cats – FVRCP, Rabies, (Leukemia Vaccine is not part of the plan.)
• Rabbits – Pasteurella, RHDV (Microchip not included and is mandatory in order to receive RHDV as per USDA rule)
• Ferrets – Distemper, Rabies
